• A 20% fee will be retained in the event of a cancellation up until May 15 after which a 50% fee will be charged.

• Free admission. Crowd estimates 3500-5000. See attached press release for event information.

• Acceptable are handmade arts & crafts or fine arts by maker: pre-packaged chocolate food items; or chocolate food vendors with open product; chocolate-related commercially produced items (crafts, T-shirts, cookbooks, etc.) are preferred however non-chocolate commercial vendors will be accepted upon approval. Non-profit organizations with no food items may apply at the lower rate; if having food items, you are a food vendor.

• If you have open food products, a Certificate of Insurance worth one million dollars and naming “ Another Bullwinkel Show & the North Shattuck Association and its members, directors, employees, agents, subcontractors, representatives, attorneys, brokers, insurers, lenders, officers, directors, partners, successors (collectively "Producers")” as additional insureds must be mailed with this application or faxed to promoter by you or your agent no later than April 17. Food vendors do not take event tickets – your sales are your own.

• If you have food products return Health Dept. forms & fees to promoter with this application.

Print out the last 4 pages then mail it to Anotheseparate check payable to the City of Berkeley.

• Submit a valid CA Sellers Permit number with this application. It may be obtained by calling the State Board of Equalization 510.622.4100. You are responsible for collsales tax. Use reporting address as 1700 Shattuck Ave., Berkeley, CA.

• Submit 2 photographs of your items for sale and 1 of your booth if available. The photos should be representative of that to be sold in your booth. Photos will be used to determine if work screened is the same as that which is in your booth. Put your name and address on the backeach photo. They will be returned day of event if possible. Some may be used in the press.

• After receiving your application and fees, promoter will assign vendors a 10’x10’ space or vendors with open food a 10’x15’ frontage on the street. Producer will issue you a badge of operation that must be worn at your booth. You may be able to parallel park behind your booth in a small car space. Other parking may be available in at a local school – more details later.

• Begin set up after 7 a.m. to be open for business at 10 a.m. Booth must remain open 5 p.m. The street is reopened 6:30 p.m. Only a long one-block area of streetfestival. The remainder of the event is on the sidewalk.

• No sales of alcohol. No Styrofoam is allowed in Berkeley. Alcohol-free event.

• Provide your own chairs, booth, lights, ice, garbage can, etc. Water is available. There is no electricity available. You may be able to park your vehicle right behind your booth.

• In case of extremely bad weather or a natural disaster the show may be canceled at the discretion of the Producer. Call 510.548.5335or payments will be made if show is canceled.
